Amendment to FADV 3-Way Agreement - Schedule B

(Effective August 1, 2011)

This amendment is required to be signed by all contractors that are currently ordering backgrounds, prior reports,
or ala carte searches through Safety Council of Texas City (SCTC). This becomes part of the originally signed
FADV 3-Way Agreement. The amendment details the requirement by the State of Texas to collect sales tax on
background checks, and as of August 1, 2011, SCTC will begin collecting the sales tax, which will increase the
price of the Background Check Graded (BCG) package and ala carte items.

Begining August 1, 2011 any company ordering backgrounds, will be checked to see if this page as been submitted on
behalf of the company. This form only has to be completed one time per company.
